What are Ciclosporine capsules (cyclosporine, cyclosporin, ciclosporin, Neoral, Sandimmune)?
Ciclosporine (can also be spelled as cyclosporine, cyclosporin or ciclosporin) is a calcineurin inhibitor and an immunosuppressant. It forms a complex with cyclophilin to block the phosphatase activity of calcineurin. By this, Ciclosporine capsules decrease the production of inflammatory cytokines by T-lymphocytes related with immunity and suppress abnormal immune reactions.
The main indication of Ciclosporine is to suppress the rejection of transplanted organ and to suppress the immune response for cell transplantation. However, many doctors all over the world use its immunity-suppressing abilities for the treatment of rheumatoid arthritis and psoriasis, severe atopic dermatitis, Kimura disease, pyoderma gangrenosum, chronic hives, acute systemic mastocytosis and acute severe ulcerative colitis. For these indications, you shall follow the instructions of your doctor.

Indications:
rejection of organ transplantation (kidney, liver, heart, lung, pancreas, small intestine), rejection/graft-versus-host disease caused by bone-marrow transplantation, suppression of immune response associated with cell transplantation.
How to take
Your doctor should prescribe the exact dosage and regimen according to your symptoms, the dosage can be adjusted at any time. Below are the general recommendations.
- Renal transplantation: 4.5-6 mg of the active ingredient per 1 kg of body weight (mg/kg) at a time, 2 times a day, from one day prior to transplantation. After transplantation the dosage will be decreased by 2 mg/kg a day. The standard maintenance dosage is 4-6 mg/kg a day.
- Liver transplantation: 7-8 mg/kg of the active ingredient at a time, 2 times a day, from one day prior to transplantation. After transplantation the dosage will be decreased gradually. The standard maintenance dosage is 5-10 mg/kg a day.
- Heart/lung/pancreas transplantation: 5-7.5 mg/kg of the active ingredient at a time, 2 times a day, from one day prior to transplantation. After transplantation the dosage will be decreased gradually. The standard maintenance dosage is 2-6 mg/kg a day.
- Small intestine transplantation: 7-8 mg/kg of the active ingredient at a time, 2 times a day. After transplantation the dosage will be decreased gradually. The standard maintenance dosage is 5-10 mg/kg a day. In general, injection form of ciclosporin is used from one day prior to transplantation, and when oral administration becomes possible, start taking the capsules.
- Bone marrow transplantation: 3-6 mg/kg of the active ingredient at a time, 2 times a day, from one day prior to transplantation and maintain the dosage for 3-6 months. After that the dosage will be decreased gradually and eventually stopped.
- For suppression of immune response associated with cell transplantation: use based on dosage and administration or usage of the cellular and tissue-based product.
This medicine contains 10 mg of the active ingredient in a capsule.
Contraindications: do not use for patients with liver/renal disorder or neuro-Behcet disease. Do not use for pregnant or breastfeeding women.
Important information
As this medicine suppresses the immunity, you should be careful to prevent the infections. Wash your hands thoroughly and gargle more often.
Avoid grapefruit juice and any food or supplements containing St. John's wort, as those may affect the effectiveness of this medicine.
If an allergic reaction occurs, patient needs to stop using the medicine and consult with their doctor. If patient is taking any other medication or treatment, they should consult with their doctor in advance.
